This program evaluation survey template gives training teams, nonprofits, and education providers a complete printed questionnaire for measuring whether a program worked. It runs 20 questions across roughly six printed pages and takes participants about ten minutes to complete on paper.
What this survey measures
The form opens with participant background, including role, experience, cohort, and attendance, so you can segment results later. Dedicated sections then rate the program content on eight quality dimensions, check whether the pace and difficulty were right, and assess the facilitators on knowledge, clarity, feedback, and time management. A before and after section asks participants to rate four core skills twice, once for how they were before the program and once for how they are now, which makes growth visible in a single grid. Final sections cover application at work, barriers to using the material, a likelihood-to-recommend score, an overall value rating, and three open questions about what to keep, drop, or change.
How to use it on paper
Hand the survey out at the closing session or mail it to participants with a return envelope. Respondents cross the boxes with a blue or black pen.
