This detailed hotel guest satisfaction survey follows the whole guest journey across six printed pages, from the moment of arrival to the final farewell at check-out. What this survey measures
The form opens with the basics of the stay: arrival date, number of nights, room number, and travel purpose. Dedicated sections then rate arrival and check-in, the room itself including cleanliness, comfort, and amenities, breakfast and restaurant dining, and facilities such as the pool, spa, gym, concierge, and Wi-Fi. A closing section covers check-out, likelihood to recommend, and intent to return, followed by two open comment boxes for highlights and suggested improvements. Every rating grid uses the same five-point satisfaction scale, so results are easy to compare across departments.
How to use it on paper
Place a copy in each room, hand one over at check-out, or include it with the final bill. Guests mark the boxes with a blue or black pen and leave the form at the front desk.
