This printable museum visitor survey helps museums and galleries hear from every guest, including the many who never respond to an email follow-up. The form prints on two to three pages with one clearly labeled section per page and takes about five minutes to complete.
What this survey measures
The survey opens with the context of the visit: whether it is a first trip, who the visitor came with, and how they heard about the museum. The experience section rates satisfaction with exhibitions, staff, signage, facilities, the shop, and the cafe, then asks which area guests enjoyed most and how they rate value for money. A final section captures how likely visitors are to recommend the museum and to return, and two comment boxes invite them to describe what they enjoyed most and what could be better.
How to use it on paper
Hand copies out at the welcome desk, leave them in the cafe, or place them near the exit with a collection box. Visitors fill the checkboxes with a blue or black pen. Collect the completed forms and scan them in batches: PaperSurvey reads the checkbox answers with optical mark recognition and converts the handwritten comments to text automatically, so results appear in your dashboard without manual data entry.
Customize this template
Add your museum logo, rename the exhibit areas to match your galleries, ask about a specific exhibition or event, or translate the form for international visitors. The paper layout reflows automatically as you edit, and every copy you print stays scannable.
