This company culture survey helps HR teams and managers understand how employees experience the workplace and where the organization can improve. It is designed for internal staff feedback and is answered anonymously so people can respond honestly. The template has 8 questions and prints on roughly two pages.
What this survey measures
The survey opens with open text questions about culture satisfaction, feeling respected, and what would make the company a better place to work, followed by a yes or no question on whether staff would recommend working here. A nine statement agreement grid covers voice being heard, benefits, feedback from management, mutual respect, recognition, celebrating success, coworker relationships, and access to resources and training, each rated from strongly disagree to strongly agree. Two short demographic questions capture job classification and length of service so responses can be grouped by employee segment.
How to use it on paper
Print the survey and distribute it during team meetings, at onboarding sessions, or through internal mailboxes wherever staff can complete it privately. Ask respondents to use a blue or black pen. Collect the finished forms in a sealed box to protect anonymity, then scan or photograph them and upload the images to PaperSurvey, which reads the marked ratings and handwritten comments automatically and compiles the results for you.
Customize this template
Most teams add their logo, adjust the agreement scale wording, and tailor the statement grid to match their own values or engagement goals. You can add or remove open text questions and update the tenure ranges to fit your workforce. The layout reflows automatically as you edit, so spacing and page breaks stay clean.
