This printable mystery shopper evaluation form gives your shoppers a structured, six-page checklist to complete right after every store visit. What this survey measures

The form opens with visit details: location, date, and exact arrival and departure times. A Yes/No compliance grid covers the exterior and entry, an eight-statement grid rates the staff interaction alongside greeting speed and the employee served by, and further grids score product presentation standards and the checkout experience, including queue time and available payment options. The final page captures a likelihood-to-recommend score, an overall visit rating, open-ended best and worst moments, and whether the shopper would return as a genuine customer.

How to use it on paper
