What this survey measures
The form is organized into three sections. A course rating grid asks participants to score whether the content met their needs, whether materials and resources were useful, whether the location was appropriate, how well the course was organized, whether the objectives were met, and whether the exercises and role play were relevant. An instructor evaluation grid covers subject knowledge, encouragement to participate, course length, and how well prepared the instructor was to answer questions. How to use it on paper
Print a copy for each participant and hand it out at the end of the session, on the way out of the room, or with the course completion materials. Ask everyone to use a blue or black pen and to mark one box per row on the rating grids.
