Course evaluations tell you how a class actually landed with students, but only if enough of them respond. Many departments moved to emailed online evaluations for the convenience, then watched participation collapse. A term with a low response rate produces numbers no committee should use to judge a course or an instructor. Designing a scannable evaluation form
A good evaluation form mixes Likert-scale items for aggregation with an open comment box for the nuance that scores miss. You can build both in the same layout and keep it on plain paper.
- Likert blocks for the standard items: Ask students to mark one bubble per row with a pen for prompts like "The instructor explained concepts clearly" on a 1 to 5 scale. These become the checkbox fields that the scanner reads automatically.
- Numeric fields where you need a figure: Estimated weekly hours or a section number can be captured as handwritten digits and recognized automatically.
- Open comment areas for written feedback: Leave a clear box for "What would improve this course?" Handwriting recognition transcribes cursive and print, so those comments arrive as searchable text rather than a pile you retype.
- Per-page identifiers keep multi-page forms intact: A two-sided or multi-page evaluation stays stitched together per respondent through unique per-page codes.

Collecting in the last ten minutes of class
The logistics are deliberately boring, and that is the point. Hand out forms, ask students to mark answers with a pen, collect them at the door, and you are done.
- Keep the instructor out of the room if policy requires it: A student volunteer can collect and seal the stack, preserving anonymity.
- One stack per section: Because each form carries its survey and page identity in a scannable code, you can mix sections during scanning and still aggregate them correctly afterward.
