This discrete choice (conjoint) survey template presents respondents with a series of choice sets and asks them to pick their preferred option, which is how conjoint analysis uncovers what people truly value. It pairs the choice tasks with a short importance rating and an open comment, and prints on one or two clean pages.
What this survey measures
Each choice set varies a few attributes, such as brand, price, contract length, and support, so you can estimate the relative weight of each attribute from the pattern of choices. The closing questions capture stated importance and any factor the fixed attributes miss.
How to use it on paper
Hand the survey out in interviews, focus groups, or by mail where a screen is not practical. Respondents cross one option per choice set with a blue or black pen. Collect the forms and scan them: PaperSurvey reads the marks with optical mark recognition so every choice is captured for analysis without manual data entry.
Customize this template
Change the attributes and levels in each choice set to match your product or service, add or remove choice sets, and adjust the number of options per set. The layout reflows automatically as you edit.
