A visitor feedback form for open days, fairs, festivals and community events. Print copies for each event straight from a list: every copy carries its event, city and collector, and those details are saved with each answer, so results can be compared by event without anyone writing them in.
How the details get onto each copy
Upload a list with a row for each event, with columns named Event, City and Collector. When you print copies from the list, each copy shows its row's details in the panel under the title and the event and city in the footer of every page, and the three Prefill Data questions save the same details with every scanned answer.
Printing for a single event? Leave the list out and add tags such as Event:Riverside open day when you print instead.
