This VVIQ printable PDF is a ready to hand out version of the Vividness of Visual Imagery Questionnaire, a 16 item self report measure of how clear and vivid a person's mental imagery is. It is used by psychology researchers, students, and clinicians studying imagery ability, including aphantasia and hyperphantasia. The instrument runs to roughly two to three pages when printed. It is based on the Vividness of Visual Imagery Questionnaire developed by David Marks.
What this survey measures
Respondents form a mental picture and rate how vivid it is on a five point scale, from no image at all to as vivid as normal vision. The 16 items are grouped into four scenarios: a familiar friend or relative, a rising sun, the front of a shop you often visit, and a country scene with trees, mountains, and a lake. Each scenario asks about specific details such as contours, colors, movement, and light.
How to use it on paper
Print the questionnaire and give it to participants in a lab session, classroom, waiting room, or as a take home form. Ask each person to visualize the scene described, then use a blue or black pen to mark one box per item. Collect the completed forms, then scan or photograph them and upload the batch to PaperSurvey.
